Endometriosis at Saint Mary's Hospital

Saint Mary’s Hospital provides a specialist Complex Endometriosis Service within gynaecology. The Joint Endometriosis Clinic is a regional service in a tertiary referral centre, managing complex endometriosis and chronic pelvic pain. The service is nationally accredited by the British Society for Gynaecological Endoscopy (BSGE) as an endometriosis treatment centre.

The service offers an excellent learning environment for international doctors seeking structured exposure to complex benign gynaecology, chronic pelvic pain pathways, advanced diagnostics, medical management, surgical decision-making and multidisciplinary care. Clinics are held at Withington Community Hospital, with the wider Saint Mary’s gynaecology service working across Saint Mary’s and Wythenshawe hospitals.

The service has a multidisciplinary team of experts to offer advanced diagnostics, medical and surgical management and offering support. This includes an endometriosis clinical nurse specialist who supports patients between appointments.

The specialty is particularly suited to international doctors who want to understand how a tertiary NHS service approaches suspected and confirmed endometriosis, including symptom assessment, imaging, fertility considerations, pain management, patient-centred communication, MDT coordination, minimally invasive surgery and governance standards. The centre participates in clinical trials and research, for example studies to prevent recurrence of endometriosis which reflects the departments commitment to improve long term outcomes.

Fellowships and observerships at Manchester Specialist Health Academy provide international clinicians with structured opportunities to experience high quality healthcare training within a leading NHS environment. Both pathways offer consultant oversight, a clear induction, and access to modern clinical settings that support a strong understanding of advanced patient care. While fellows take part in more in depth clinical development, gaining exposure to subspecialties, procedures and research, observers focus on shadowing and understanding multidisciplinary practice within NHS services. Both groups benefit from involvement in clinics, team discussions, and wider learning activities such as simulation sessions, CPD opportunities, and quality improvement insight.

Rare and complex cases

Visitors may gain exposure to complex endometriosis and chronic pelvic pain presentations that require specialist assessment and coordinated decision-making. Examples include:

  • Deep infiltrating endometriosis and complex pelvic pain.
  • Endometriosis associated with bowel, bladder, vaginal or rectal symptoms, where multidisciplinary input may be required.
  • Patients with fertility concerns or reproductive medicine considerations.
  • Cases requiring decision-making around ultrasound, MRI, diagnostic laparoscopy, medical treatment and/or laparoscopic or robotic surgery.
  • Adolescent or young adult presentations where diagnosis, communication and longer-term quality of life are particularly important.
  • Patients with persistent or recurrent symptoms, previous failed treatment or suspected disease involving adjacent organs.

Observership
Endometriosis

Objectives

By the end of the placement, the visiting professional should be able to: 

  • Describe the structure and role of a nationally accredited specialist endometriosis service managing complex endometriosis and chronic pelvic pain.
  • Observe how patients with suspected or confirmed endometriosis are assessed, supported and counselled around diagnosis and treatment options. 
  • Gain awareness of common symptoms and clinical presentations of endometriosis, including pelvic pain, painful periods, fertility concerns, bowel symptoms and bladder symptoms. 
  • Understand the contribution of multidisciplinary team members, including consultants, Clinical Nurse Specialists and Advanced Clinical Practitioners, in delivering coordinated care. 
  • Observe how imaging, including MRI, can support assessment of deep and complex endometriosis and assist with surgical planning. 
  • Reflect on patient-centred approaches to long-term condition management, shared decision making and support for quality of life in endometriosis care.

Eligibility

  • The visiting professional must have sufficient English language skills to engage meaningfully in the observership.

Objectives

By the end of the placement, fellows should be able to:

  • Develop advanced understanding of the assessment and management of complex endometriosis and chronic pelvic pain within a regional tertiary referral service.
  • Gain supervised exposure to multidisciplinary endometriosis care, including advanced diagnostics, medical management, surgical planning and ongoing patient support. 
  • Understand the role of Clinical Nurse Specialists and Advanced Clinical Practitioners in coordinated care, patient education, treatment planning and multidisciplinary team working. 
  • Build familiarity with investigation pathways for suspected deep or complex endometriosis, including the role of pelvic MRI in diagnosis and surgical planning.
  • Observe the principles of complex endometriosis surgery, including surgical excision, management of disease affecting bowel, bladder, ureters, ovaries and surrounding pelvic structures, and the importance of pre-operative planning.
  • Reflect on how specialist endometriosis pathways, multidisciplinary working and patient-centred counselling could inform service development in the fellow’s home healthcare setting.

Eligibility

Postgraduate Qualification: Applicants must hold an overseas-recognised postgraduate medical qualification (MD, MS, Masters) in obstetrics and gynaecology, fetal medicine, maternal-fetal medicine or a closely related specialty.

Experience: Minimum of 3 years’ recent experience in obstetrics and gynaecology or related specialty within the last 5 years, including at least 12 months of continuous practice before applying.

Training Level: Equivalent to ST5 or above, or post-CCT clinicians seeking subspecialty exposure.

Language Proficiency: IELTS Academic with 7.0 in each band and overall 7.5, or OET Medicine with Grade B in all domains.

Registration: GMC registration is required for fellows.

Demonstrable prior experience in obstetrics and gynaecology, with interest in endometriosis, chronic pelvic pain, reproductive medicine, benign gynaecology, minimally invasive surgery and/or adolescent gynaecology.

Evidence of safe clinical practice, professional standing, and commitment to patient-centred care.

For any surgical fellowships, a logbook or equivalent evidence of laparoscopic experience may be requested.

Any access to theatres, MDTs, imaging meetings, research projects or direct clinical duties will be subject to local governance, supervisor approval and capacity.
